Free & Reduced Meal Application 2014-2015

Please use this link to access the 2014-2015 Free & Reduced Meal Application. You will only need ONE APPLICATION PER HOUSEHOLD for your children in the Victor Elementary School District. Applications submitted online will be received by the Nutrition Service Department automatically.

Special Meals and/or Accommodations

This form is to be when: Participant has a disability or a medical condition and requires a special meal or accommodation. Participant does not have a disability, but is requesting a special meal or accommodation due to food intolerance(s) or other medical reasons.

Religious Belief Form

Please fill out this form if your child should avoid certain foods due to religious beliefs.

Pizza Party Form

Please submit two weeks prior to event.

Temperature Logs

Lead Cooks, you may access a copy of the temperature log used to monitor your milk box, refrigerator and freezer.
